Sports Camp - August 2026

Radiant Homeschool Collective, Inc. (“Radiant”) is organizing a multi-sport camp (“Sports Camp”) for homeschool students. Camp activities may include but are not limited to running, jumping, throwing, catching, and participation in various team and individual sports and athletic drills. The Camp will be held at various parks and outdoor locations in the Kansas City, Missouri area every Monday in August 2026. Assumption of Risk I understand and acknowledge that participation in sports and physical activities involves inherent risks of injury, including but not limited to: • Sprains, strains, and muscle injuries • Fractures and broken bones • Cuts, scrapes, and bruising • Head, neck, or spinal injuries • Heat exhaustion or dehydration • Injuries caused by contact with other participants or equipment • Injuries resulting from uneven terrain, outdoor surfaces, or weather conditions • Other injuries, both foreseeable and unforeseeable I voluntarily assume all risks associated with my child’s participation in the Camp, whether those risks are known or unknown, and whether caused by the negligence of Radiant, its board members, volunteers, or other participants. Release of Liability In consideration of my child’s participation in the Radiant Sports Camp, I, on behalf of myself and my child, hereby release, waive, and discharge Radiant Homeschool Collective, Inc., its board members, officers, volunteers, and agents (collectively, “Radiant”) from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury — including death — that may be sustained by my child during participation in the Camp or while traveling to or from Camp activities. This release applies to claims arising from ordinary negligence and does not apply to claims arising from gross negligence or willful misconduct. Indemnification I agree to indemnify and hold harmless Radiant Homeschool Collective, Inc. and its board members, officers, and volunteers from any loss, liability, damage, or cost they may incur as a result of my child’s participation in the Camp, including claims brought by third parties arising from my child’s conduct during Camp activities. Participant Conduct I agree that my child will: • Follow all instructions given by camp leaders and volunteers • Treat all participants, leaders, and volunteers with respect • Refrain from behavior that could endanger themselves or others • Report any injury or unsafe condition to a camp leader immediately I understand that my child may be removed from Camp activities without refund if their conduct is unsafe, disruptive, or inconsistent with Radiant’s Code of Conduct. Acknowledgment By signing below, I acknowledge that: • I have read this waiver fully and understand its contents • I am the parent or legal guardian of the participant named above and have the authority to sign on their behalf • I voluntarily agree to all terms stated in this waiver • I understand I am waiving certain legal rights by signing this document
